This See&Treat project in the slump area of Jakarta and rural areas in Tasikmalaya (Bandung), and Bali consists of screening women for cervical cancer and pre-malignant lesions using Visual Inspection with Acetic Acid (VIA) and the Pap Smear test, offering immediate treatment with cryotherapy and where appropriate referring women for gynaecologic evaluation and treatment at the secondary or tertiary care levels. The target population consists of sexually active women in the age 25-50 year.

In October 2004 The Female Cancer Program has started the See&Treat in three regions in Indonesia. The target is to screen and provide follow-up services if needed to 22.000 women over the period of October 2004 - July 2006 (also know as demonstration project). The mission of this project developed by the Female Cancer Program and her local partners is to create a sustainable and cost-effective method for the screening, downstaging and treatment of cervical cancer and to raise the awareness about sexual reproductive health. Our local partner the Academic Centres in Jakarta, Bandung and Bali, the Indonesian Cancer Foundation YKI and Family Welfare Program of Indonesia PKK are essential to fulfil our mission.

The See&Treat teams work daily in a team that consists of two doctors and four public health workers. In special designed cars this team visit each day the public health clinics in the different villages and screen and treat about 50 women. To accomplish maximal impact in each sub district we have an 80% screenings target. To reach this target we have set up awareness campaigns about cervical cancer and sexual reproductive health in cooperation with the Family Welfare Program of Indonesia, the PKK and the Indonesian Cancer Foundation YKI.

The PKK is structured from national level to sub village level in city and remote areas. All this levels have their own board in which the chairman is a women married to the Minister, Governor, Mayor, Bupati until the lowest level. This organisation is the infrastructure link for the See&Treat. Also different policy makers from the Ministry of Health to sub district level are being informed and involved in the project. This organisation strongly supports the awareness about sexual reproductive health and is in favour to expand the awareness. They PKK pointed out that while women waiting in public health centres for screening, there is an ideal opportunity to inform the women on other reproductive health aspects.

The YKI as National Cancer Foundation is in charge in prevention programs in all different sorts of cancer. The services do not consist only of providing information, but also to provide training for general practitioners midwifes/nurses and the necessary drugs for chemotherapy against minimal prizes. For training the YKI uses the "Train the Trainer" principle. In the awareness services we will meet Indonesian womens cultural, emotional and practical needs, which is essential to the success of awareness.

At the end of the demonstration project the relation between cytology and VIA will be compared and the usefulness of the relatively easy and cost-effective technique as an alternative screening method for low resource areas will be evaluated. At the same time the satisfaction and the acceptability of the programme will be evaluated. The national evaluation will take place in august 2006.
